Misconduct Records for Current and Former Students

The Office of Student Conduct and Community Standards maintains digital records of nonacademic and academic misconduct (“disciplinary files”). Current students, former students, and alum have the right to inspect their record, consistent with federal law. Records are often requested to fulfill reporting obligations when applying for graduate/professional school, professional licensing, transferring schools, etc. Please see the directions below about how to request records.

If you are looking for information about your disciplinary file and/or need a Dean’s letter/certification* sent on your behalf please submit a request using the online form here.

We will attempt to fulfill requests within three business days (excluding administrative closures). We encourage you to plan ahead to make sure your requests are submitted in time for your application deadlines.

If you are looking for information related to your disciplinary record and/or a Dean’s letter/certification* sent on your behalf, please schedule a 15 minute appointment (phone or in person) with any conduct professional staff member by emailing The purpose of this appointment is to verify your identity and to obtain information about the request. After this conversation, the conduct staff member will complete any follow up related to that appointment.

*Information about Dean’s Certifications and Other Third-Party Disclosures

We complete Dean’s Certifications and other records checks for student disciplinary histories. We will complete the following types of disclosures:

    • Uploading to online application portal, such as AMCAS, provided the applicant supplies us the necessary information
    • Email disclosures directly to third party (admissions offices, law enforcement agencies, etc.)
    • Fax disclosures directly to third party
    • Postal mail disclosures directly to third party. Applicant must provide us with a self-addressed, stamped envelope. Please deliver envelopes to:
