Current students, former students, and alum have the right to inspect their record, consistent with federal law. Records are often requested to fulfill reporting obligations when applying for graduate/professional school, professional licensing, transferring schools, etc. Please see the directions below about how to request records.

If you are looking for information about your disciplinary file and/or need a Dean’s letter/certification* sent on your behalf please submit a request using the online form here.
We will attempt to fulfill requests within three business days (excluding administrative closures). We encourage you to plan ahead to make sure your requests are submitted in time for your application deadlines.
If you call or walk in, unless a staff member is available, you will be directed to schedule an appointment with a staff member from the office. This appointment can be made by emailing conduct@studentlife.wisc.edu.
Individuals without access to MyUW
If you are looking for information related to your disciplinary record and/or a Dean’s letter/certification* sent on your behalf, please schedule a 15 minute appointment (phone or in person) with any conduct professional staff member by emailing conduct@studentlife.wisc.edu. The purpose of this appointment is to verify your identity and to obtain information about the request. After this conversation, the conduct staff member will complete any follow up related to that appointment.
